春秋晉景公年間，奸臣屠岸賈(王學圻 飾)欲除忠烈名門趙氏一族。惟一漏網的，就是身懷六甲的趙朔妻子庄姬公主(范冰冰 飾)。大夫程嬰(葛優 飾)受恩於趙氏夫婦。為報知遇之恩，程嬰冒?生命危險把庄姬剛生下的男嬰，偷偷地帶到自己家中。其後屠岸賈下令封城，誓要捉拿趙氏遺孤。程嬰為存道義，不惜把自己初出生的孩子拿來調換；最後程嬰的親生孩子和妻子都因此而死，只剩下程嬰和趙氏孤兒獨活下來…
光陰飛逝，趙氏孤兒已經長大成人。為報血海深仇，程嬰不惜安排趙武(長大後的趙氏孤兒)與屠岸賈結為乾爹乾兒子的關係；而自己則與另一趙氏忠臣韓厥(黃曉明 飾)日夜相議復仇大計…蒼天不負有心人，程嬰在韓厥裡應外合下，終於能發動計劃已久的復仇行動，勢要消滅仇人屠岸賈。國仇家恨，雖能一朝了斷；但十五年的喪子之痛，又會否隨?手仞仇人而得以忘懷！？
